Submitting your PMC Report

There are three ways to submit the Professional Maintenance and Certification (PMC) report.

Microsoft Excel:

This version of the PMC report is an Excel spreadsheet with linked cells. To download and use a copy of this report, right-click here and select "download file" or "Save Target As". It is important that you do not try to run this file from its present location, as that will not work and might cause your report to become unstable. Please note that some of the cells are password protected. If you need assistance on filling out this report, please contact rharris@entsoc.org.

PDF:

The report is due on March 15^th of the year following the third year covered in the report (e.g., a report due on March 15^th, 2007 would cover 2004, 2005, and 2006). For new BCEs, if you were certified in the first half of a year, your "start date" for accumulating continuing education unites (CEUs) would be January 1st of that year. If you were certified in the second half of the year, your start date for CEUs would be January 1st of the following year. Your PMC report should be submitted to the following address:
